STUDENT HEALTH @American Baptist College
Health Insurance Plan
All students enrolled at the college are required to have health insurance.
American Baptist College is pleased to announce that in 2014-2015 Academic Year we teamed up with University Health Plans and Nationwide Life Insurance Company to provide health insurance coverage for our students.
The Student Health Insurance Plan is effective 9/1/2014 through 8/31/2015. The annual premium is $2,073. American Baptist College’s policy requires all students registered in 9 or more credits to have health insurance. Unless proof of comparable coverage is provided, you must participate in the American Baptist College Student Health Insurance Plan.
If you do not submit an online waiver form by the deadline below, you will automatically be enrolled in the College's Student Health Insurance Plan and you will be responsible for the cost of the health insurance premium.
If you submit an insurance waiver and then lose your coverage due to a qualifying life changing event, you must contact University Health Plans 800-437-6448 within 60 days to be enrolled in the Student Health Insurance Plan.
All international students must accept the College's insurance program unless they have a special exception by proving they have a US-based insurance plan that is comparable to the Student Health Insurance Plan.
Please contact University Health Plans with any questions, 1-800-437-6448.
The Health Insurance Waiver Deadline for the 2014-2015 academic year is September 30, 2014.
In an attempt to maintain a safe and healthy campus environment, American Baptist College requires that all entering students, born after 1957, furnish documented proof of having immunity to measles or having been immunized with two doses of MMR vaccine on or after the first birthday unless contraindicated because of pregnancy, allergy to a vaccine component, or other valid medical reason(s). The law states that immunizations are not required if they conflict with the parents, or guardians, (or individuals over 18) religious tenets and practices, affirmed under the penalties of perjury. They are also not required if a qualified physician shall certify that administration of such immunization would be in any manner harmful to the child involved. (TCA Section 49-6- 5001). The Certificate of Immunization must be completed and signed by a licensed M.D. or D.O. and returned to the American Baptist College Student Health services.
NOTE: YOU WILL NOT BE ALLOWED TO REGISTER FOR CLASSES UNTIL AN ACCEPTABLE FORM IS ON FILE IN THE STUDENT HEALTH SERVICES OFFICE.
